Two issues with Smiths Medical CADD Infusion System Infusion Sets

The first issue may occur due to manufacturing variations that cause a partial occlusion of the tubing. The second issue is specific to CADD-Legacy pumps, which have been discontinued. The pumps may not detect the 50 ml and 700 ml CADD Medication Cassette Reservoirs with Flow Stop and initiate a NOA alarm. The user must resolve the alarm before using the pump. This issue does not affect 250 ml Flow-Stop and non-Flow Stop CADD Medication Cassette Reservoirs.

The patient impacts of the two potential issues with the CADD Infusion System Infusion Sets are as follows:

  1. Lack of delivery or under delivery: If the green CADD Flow Stop arm compresses partially occludes the tubing before use, there is a possibility of under delivery or non-delivery of medication, which can lead to a potentially life-threatening situation for patients who rely on the infusion system for life sustaining therapy.
  2. False "no disposable attached" (NOA) alarms: The false NOA alarms in CADD-Legacy pumps can cause confusion and delay in the delivery of medication, which can result in harm to the patient if the NOA double-beep warning is not resolved within 2 minutes. The user must clear the alarm and resolve the cause of the NOA event, which can add additional stress and pressure to the healthcare provider and patient.
